People Keeping Well in their Community

‘People Keeping Well in their Community’ is a community based prevention initiative that aims to delay the need for health and social care services. As part of Sheffield’s approach to Social Prescribing, it focuses on resolving social issues and connecting individuals with local resources that matter to them, helping to reduce the risk of poor health and wellbeing.

Our Role

At Stocksbridge Community Leisure Centre, we are dedicated to supporting this initiative by providing various services to enhance physical, mental, and social well-being, which is why we are the lead body for People Keeping Well in the Community across Stocksbridge and the Upper Don.

Social  prescribing

Social prescribing connects individuals to non-clinical community support, addressing root causes of health issues like isolation, financial stress, or housing insecurity.

Community wellbeing coaching

One-to-one coaching helps residents adopt sustainable health behaviours through personalised plans.

Why are our services important?

Tackling the root causes of poor health
Many health challenges—such as loneliness, inactivity, or stress—stem from social and lifestyle factors. Our services, including social prescribing, community engagement, and wellbeing coaching, address these root causes by providing practical support and opportunities for meaningful connection.

Through partnerships with local GPs and community organisations, we help residents access a wide range of activities and support networks.
